Guide

How to claim a medical reimbursement from Sukoon

To claim a medical reimbursement from Sukoon, fill in the Sukoon claim form, have your doctor complete, sign and stamp the medical section, and submit it with your original invoices and reports through the mySukoon app or medical.sukoon.com, at a branch, via your broker or by courier, within 120 days of treatment. Sukoon aims to pay eligible claims within 3 weeks.

  1. Enter the claimant details

Add your full name, card number, phone number and email address exactly as they appear on your Sukoon card. Every field is optional: you can open the PDF at any point and finish the rest by hand. Each person claims separately: Sukoon allows one claim submission per person, so family members each need their own form.

  2. Add the claim details

Enter the hospital or doctor's name, the date of treatment and the amount you are claiming. You can leave the amount empty if you do not have it yet.

  3. Download the PDF and have your doctor complete it

The treating doctor completes the medical details, then signs and stamps the form. Sukoon asks for a form completed by you and the doctor for each visit or course of treatment. You can enter the medical details and optional bank details in this tool first. The claimant name is repeated in the declaration and on the last page. Complete the signatures, stamps, declaration dates, invoice count, invoice summary and checklist by hand.

  4. Submit the claim to Sukoon

Upload it in the mySukoon app or at medical.sukoon.com, take it to any Sukoon branch, send it through your broker, or courier it to the Medical Claims Department in Deira, Dubai. Sukoon says to submit within 120 days of the treatment. Keep a copy of everything you send.
